What is the purpose of this survey?   To understand how you find (or would find) information about mental health, addictions, and gambling services.

I don’t have a mental health and/or addictions issue and I don’t know anyone who does.   If you have not experienced problems with mental health, addictions, or gambling, and do not know anyone who has, your opinions are still very important.  MOHLTC would like to understand where you would go to get information or services and your awareness of other services in the area of mental health, addictions, and problem gambling.

Will my survey answers affect the services myself or my family/friend(s) get?    No, what you say in the survey will not affect services you or your family/friend(s) are receiving. The survey is being conducted by an independent firm, Malatest, to make sure that your responses are confidential.

What will MOHLTC do with the information?    Your survey answers will be summarized along with responses from other individuals who participate in the study.  Your individual responses will be completely confidential and will not be shared.
